As our client, you will receive an individualized psychological assessment. We have over 50 years of combined experience conducting psychological evaluations.
Our evaluations assess for Neurodevelopmental Disorders, Schizophrenia Spectrum & Other Psychotic Disorders, Bipolar & Related Disorders, Depressive Disorders, Anxiety Disorders, Obsessive-Compulsive & Related Disorders, Trauma- and Stressor-Related Disorders, Neurocognitive Disorders, and Conduct Disorders. We strive to be accessible to all individuals.

Dr. Smith has extensive experience assessing for Autism Spectrum Disorders, ADHD, and Intellectual Disabilities. Dr. Smith ONLY evaluates individuals 16 years old and older.
Dr. Sigler has extensive experience assessing for neurocognitive/cognitive disorders, Intellectual Disabilities, Adaptive Behavior Functioning, and Autism Spectrum Disorders. Dr. Sigler conducts Psychological Evaluations for the USCIS form N-648 form. Dr. Sigler ONLY evaluates 1) nonverbal &/or English Language Learners (ELL) 6-years-0-months to 16-years-11-months old and 2) most individuals 17-years-old and older.

Dr. Sigler conducts Psychological Evaluations for individuals 6 - 16 years older who are nonverbal/minimally verbal and/or non-English proficient.
Dr. Sigler also evaluates most individuals 17 years and older; this is based on the referral question.
Dr. Sigler specializes in psychological testing for neurocognitive/cognitive disorders, Intellectual Disability, Autism Spectrum Disorder in children, most psychiatric disorders such as mood, anxiety, and posttraumatic stress.
Dr. Sigler conducts Psychological Evaluations to determine eligibility for completion of a USCIS form N-648.
Dr. Sigler has extensive experience and training working with newly arrived refugees, immigrants, geriatric and GLBTQ+ individuals.

Dr. Smith earned his Doctoral Degree as a Clinical Psychologist from Wayne State University in 1988. He works with individuals ages 16 years old and older.
Dr. Smith specializes in psychological testing for Autism Spectrum Disorder, ADHD, intellectual disabilities, school accommodations, personality/psychiatric disorders and Social Security appeals evaluations.
In individual therapy he uses a Cognitive-Behavioral Skills Training and/or behavioral approach and works with people with various types of anxiety, depressive and adjustment disorders, Autism Spectrum Disorder, intellectual disabilities and ADHD. He works closely with parents, teachers, physicians and other providers. Dr. Smith is experienced in working with language interpreters and culturally diverse populations.

Individuals diagnosed with Autism Spectrum Disorder have deficits in social communication and social interaction, as well as restricted patterns of behavior, interests, and activities that interfere with their daily functioning.
Many individuals with these issues have never been diagnosed and may be wondering why they are having so many problems adjusting to their life circumstances. Getting a comprehensive and accurate diagnosis is a first step in understanding if you actually have Autism Spectrum Disorder or some other type of communication disorder.
At Diverse Assessments and Treatment, as licensed psychologists we use widely accepted psychological evaluation methods with well-established norms, we consider unique gender and cultural identities in our interpretation of the psychological test data. When a client comes in for an ASD evaluation they can expect to be assessed with cognitive/intellectual scales, rating scales, adaptive functioning scales, and a thorough gender/culturally relevant diagnostic assessment interview.

Posttraumatic Stress Disorder is a specific diagnosis in the broader diagnostic category labeled, Trauma and Stress Related Disorders. There is no single psychological evaluation for PTSD. A psychologist can evaluate you for PTSD. Not every traumatized person develops ongoing (chronic) or even short-term (acute) PTSD. Not everyone with PTSD has been through a dangerous event. Some experiences, like the sudden, unexpected death of a loved one, can also cause PTSD. Symptoms usually begin early, within 3 months of the traumatic incident, but sometimes they begin years afterward. The course of the illness varies. Methods used to assess for PTSD include comprehensive diagnostic interview, use of a standardized PTSD symptom rating scale, administration of psychological test(s) to evaluate cognitive functioning and severity of symptoms and make recommendations for mental health services.

Intellectual disability is a specific diagnosis in the broader diagnostic category labeled, Neurodevelopmental Disorders. To assess an individual for an intellectual disability; Drs. Sigler & Smith, Psychologists will conduct a comprehensive diagnostic interview including developmental history information from a knowledgeable reporter, administer psychological test(s) to evaluate cognitive abilities (WAIS/WISC), adaptive behavior (e.g. Vineland). This assessment requires participation from immediate family members or caregivers or significant others to complete the adaptive behavior rating scale and provide information about the individual's pre- and post-natal period, infancy, toddler, and childhood, and provide collateral information on current functioning. A good psychological assessment is a good first step in obtaining the mental health services needed.

Permanent Residents of the US, Green card holders, become citizens through the Naturalization process. During a Naturalization interview, an applicant must demonstrate an understanding of the English language, including an ability to read, write and speak words in ordinary usage. They must also demonstrate a knowledge and understanding of the fundamentals of the history and principles and from of government of the United States.
Applicants who are unable to meet the English & civics requirements due to physical or developmental disability or mental impairment may seek an exception by submitted a USCIS form N-648. Dr. Sigler conducts a comprehensive Psychological Evaluation to assess whether an individual meets the criteria for the Medical Waiver for Disability Exceptions (USCIS form N-648)

Psychological Evaluations billed to your Health Insurance must meet "Medically Necessary" criteria established by your health insurance. A Psychological Evaluation is generally considered "Medically Necessary" when the request comes from another service provider who intends to use the written report for treatment, diagnostic clarification, medication management, service planning, and/or eligibility determination(s).
Examples of Psychological Evaluation which are NOT "Medically Necessary" and NOT covered by Insurance include:
i) Court Ordered
ii) Evaluations for to assess eligibility for the completion of USCIS form N-648: Medical Certification for Disability Exception
iii) To apply for or appeal a Social Security Disability Determination.
iv) To determine the presence of a learning disability.
v) When the Mental Health benefits for the patient have been used for the calendar year.
Scheduling/Length
Psychological Evaluations conducted by Dr. Sigler are scheduled for 1 appointment which is scheduled for 4 hours.
Psychological Evaluations conducted by Dr. Smith are scheduled as 2 appointments,, the first being the diagnostic interview, the second is the psychological testing session.
The Psychological Evaluations being conducted by Dr. Sigler and Dr. Smith CANNOT be conducted remotely. Due to the nature of the kind of testing conducted, all Psychological Evaluation appointments are scheduled as face-to-face at our clinic in Roseville, MN.
Some Health Insurances will require a Prior Authorization (PA) for a Psychological Evaluation. This means the evaluation must be schedule as 2 appointments at least 12 business days apart. The first appointment is to conduct the Diagnostic Interview and the second to administer the authorized psychological testing.
Out billing agency, JB Billing Services, LLC, will conduct a benefit check with the insurance company. JB Billing will gather information regarding your coverage for the services, whether you have a co-pay, deductible, co-insurance, or other costs which will be billed directly to you.

All new patient requests for a psychological evaluation start with completing a one-page form. There are two different intake forms for Dr. Sigler:
· One form is for new patients seeking a Psychological Evaluation to determine eligibility for the completion of a Medical Certification for Disability Exceptions (USCIS form N-648).
· The other form is for new patients requesting a Psychological Evaluation at the request of the county. This applies to individuals applying for county services like Rule 185, DD Waiver, CADI Waiver, etc.

YES. This evaluation is conducted in one 4-hour appointment.
The Psychological Evaluation consists of two parts. A Diagnostic Interview and the administration of psychological tests to the patient and a family member or close friend.
Insurance does NOT cover the cost of the N-648: Medical Certification for Disability Exception Psychological Evaluation because it does not meet “medically necessity” criteria. The cost of the Psychological Evaluation and completion of the USCIS form N-648 is $900.00. Payment is due at the time of service. Payment methods accepted include cash, cashier’s check, and credit card, and contactless platforms.
A family member or friend can (preferred) act as interpreter. Often, the family member or friend who interprets also responds to test question questions which are described below. If the interpreter is not someone who is familiar with the intended patient a family member or friend should attend so they can answer the questions.
The intended patient CANNOT attend the appointment alone. Often one of the concerns about the intended patient is their CHANGING/DECLINING MEMORY. Interviewing someone who is familiar with the person’s changes in memory is an important part of this type of Psychological Evaluation.

Dr. Sigler conducts Psychological Evaluations in a single appointment, typically scheduled for 4 hours. This time includes both the interview and testing; however, the duration can vary by appointment.
If comprehensive records are provided at least one week prior to the appointment, the actual appointment time may be shorter. On average, a Psychological Evaluation appointment lasts about 3 hours.
The need for a knowledgeable informant in a Psychological Evaluation depends on the evaluation's purpose. If assessing for:
· Intellectual Disabilities
· Autism Spectrum Disorders
· Attention-Deficit/Hyperactivity Disorder
· Other childhood and developmental disorders
· Memory issues
YES, an informant must attend to provide details on early life history. This assessment can be done via computer, paper, or face-to-face interview. Additional assessments for the informant may be required during the diagnostic interview.
Yes, I require past records prior to the appointment. The New Patient Welcome letter will outline the types of past records necessary and the methods to submit them.
Past records are essential for Psychological Evaluations. Certain diagnoses may not be possible without sufficient developmental history information.
I can provide a blank Release of Protected Health Information form to assist you in requesting past records be sent to me.
Important: You will need to mail or fax the release forms to the respective providers, clinics, or schools. While I can supply the form and relevant instructions, it is your responsibility to ensure that the records are received by me before the appointment.
To schedule a Psychological Evaluation with a language interpreter, email or leave a voicemail with your name, the patient's name, and the required language.
You may also text me in your primary language. If my translation app supports it, I will respond through the app.
If you cannot read, I can send you an audio message in your language if the app has it.
I will call you back with an interpreter or text you using the translation app to answer your questions and schedule an appointment.
If you need a language interpreter for the Psychological Evaluation, I will request one from a certified local agency.
Your insurance company will be billed if the evaluation meets Medical Necessity criteria.
You may choose a male or female interpreter.
Please note that evaluations may sometimes need to be rescheduled due to interpreter availability.
A Psychological Evaluation consists of two main components: the Diagnostic Interview and Psychological Testing.
During the Diagnostic Interview, I will gather historical information regarding pregnancy, birth, development, educational background, and mental health behaviors and symptoms from the patient's past, unless this information has already been documented in the provided records.
In most Psychological Evaluations, the patient will undergo an IQ test. This face-to-face assessment typically lasts approximately one hour.
Additional tests may be administered, including those assessing executive functioning, personality, expressive and receptive language, and other areas relevant to the referral questions.
The selection of tests is informed by the information obtained from the records, the diagnostic interview, and the specific referral questions.
